Post-Acute Transitional Care Utilizing the Swing Bed Program: Understanding the Quality and Financial Impact to Your Hospital 

OVERVIEW

Rural residents across the country have access to fewer healthcare services, lower economic and insurance status, fewer physicians per capita, and higher chronic disease rates than their urban peers. More rural hospitals have closed recently than in the prior 15 years combined (Morgan, 2014). Maintaining long-term viability of Critical Access Hospitals (CAHs) will be essential for rural healthcare delivery. Furthermore, 20% of Medicare patients are hospitalized each year and almost half require care after being discharged from the hospital to assist them in their recovery from serious illnesses. 

Participants will learn proven strategies to help CAHs thrive by establishing Transitional Post-Acute Care Programs from Allevant, developed by Mayo Clinic and Select Medical.

PROGRAM OBJECTIVES

MEET YOUR FACULTY 
Mark Lindsay, MD., is an Assistant Professor of Medicine at Mayo Clinic College of Medicine. He has practiced at Mayo Clinic Health System Eau Claire since 1997. He served as Quality Officer for Mayo Clinic Health System from 2006-2010 supporting quality, patient safety and service excellence for 19 hospitals and 70 clinics in Minnesota, Wisconsin, and Iowa. His master’s project from the USC School of Business was the development of 11 Transitional Care programs in Minnesota, Wisconsin and Iowa. He presently serves as Medical Director for Allevant supporting rural healthcare post-acute pathways. 

Monica Bourgeau, MS, is Vice President of Business Development for Allevant Solutions. She has more than twenty years of experience in healthcare leadership positions, including nearly ten years leading national rural healthcare transformation programs for CAHs. She has also held executive-level positions at a health insurance company/third party administrator and a regional managed care/population health company, in addition to serving on the board of a CAHs.
