Whether you are new to readmissions, or have been working to reduce readmissions for years, this workshop will help you gain clarity on how to take the next steps to strengthen your efforts to get better results. The ASPIRE method has been disseminated to readmission teams nationally, and is a systematic method to design and deliver whole-person transitional care - applicable to small rural hospitals and large teaching hospitals, applicable to any target population you might be working on. Come ready to interact with other readmission reduction teams and develop specific ideas relevant to your work to implement in the next week, month, and quarter.

ASPIRE to reduce readmissions: An interactive readmission reduction workshop
This one-day workshop will help readmission reduction teams strengthen their efforts to reduce readmissions.

Target Audience: Readmission reduction teams, including executives, administrators, quality and data, physicians, nurses, case managers, social workers 

Objectives for the A.M. Session:
º Design a data-informed readmission reduction strategy.
º Develop a multi-faceted portfolio of strategies.
º Develop interventions that address the root causes of readmissions for your target population(s). 

Objectives for the P.M. Session:
º Develop an operational dashboard. 
º Develop 3 action plans to guide your work over the next 1 week, 30 days, and 90 days.
