real-time email or text notifications to the
physician or the physician’s office about
ED visits, inpatient admits, inpatient
discharges, and other events for
established patients. Each provider and
office user can customize notifications as
desired to meet their needs.

Care Continuity is more than simply a notification solution. Care Continuity is the foundational platform to
support the journey from Fee for Service to Value Based Care. In the near future, Care Continuity will add
clinical documents and workflows, serving as the State's Health Information Exchange (HIE)
