What is Care Continuity and why is it important to my patients and my practice?

Care Continuity is an initiative to improve patient care as your patients visit the hospital. The Care Continuity platform is available to physician offices as a solution to notify your office when your patients register and are discharged from the hospital (inpatient, ED or outpatient). All notifications are customized to meet the individual preferences of your physician office. 

Physicians can receive notifications directly,

and can also define as many proxy users as they wish. Separate profiles can be established for practice coordinators who may require different types of notifications. In addition to real-time text and email notifications, a daily compilation is available via email, providing large practices with a convenient alternative for individual alerts.

Getting started is easy!

Our physician relationship managers are available to assist you and your designated users with the setup process. After initial enrollment, new users can easily be added or removed, and notification permissions changed through our Care Continuity portal.
