OVERVIEW:

Severe sepsis kills more than 700 people a day in the United States. Many organizations have initiated programs around the septic patient that falls short of desired outcomes. As of October 2015, the Centers for Medicare and Medicaid Services has made sepsis a core measure. The complexity of the disease, the variability in presentation of the patient, and skill and knowledge level of the provider makes this core measure distinctly different and potentially a greater challenge to implement. In order to provide the right care to the right patient at the right time across the continuum, systems and processes need to be well defined. This workshop will focus on taking the learner through a four-tiered strategy for ensuring a robust program to identify patients early in the disease and manage them based on the latest evidence. Key implementation strategies for hard-wiring the clinical and process behaviors will be revealed. Barriers with reliable actions to reduce their impact will be described and participants will have the opportunity to create test of change action plans to help address those barriers.

OBJECTIVES:

Part 1: Sepsis Update 2020: Incidence, Mortality and Bundle Science Update
       Determine the impact sepsis has on mortality, location of disposition and
          long-range economic impact; and

       Examine any new evidence on the sepsis bundles and implementation.

Part 2: Getting the Team to Play: Strategies for Increasing Team Member Buy-In
  • Examine methods to increase physician participation to ensure a comprehensive approach to recognition and management of the septic patient;

  • Determine strategies to obtain leadership buy-in for the need and actualization of a sepsis coordinator role; and 

  • Using test of change framework, identify and work on an intervention for physician buy-in that you can start by next week to implement.

Part 3: Finding the Septic Patient: Challenges and Solutions to Early Identification

  • Identify locations and processes for screen by the front line; and

  • Evaluage secondary recognition strategies and the role of the rapid response in early recognition

Part 4: Addressing the Challenges

  • Examine the evidence and practical application for fluid challenges; and

  • Identify strategies to improve timeliness of antibiotic, blood cultures and lactate.

Part 5: Septic Shock: Vasopressors and Precision Driven Fluid Resuscitation

  • Discuss rationale for use of central lines in patients with septic shock; and

  • Compare and contrast the methods to assess volume status (CVP/SCVO2, focus assessment,

          Dynamic measures of perfusion).

MEET YOUR FACULTY:
Kathleen M. Vollman MSN, RN, CCNS, FCCM, FCNS, FAAN
Vollman is a critical care clinical nurse specialist and consultant. She has published and lectured nationally and internationally on a variety of pulmonary, critical care, prevention of healthcare acquired injuries, work culture and sepsis recognition and management. From 1989 to 2003, she functioned in the role of clinical nurse specialist for the Medical ICU's at Henry Ford Hospital in Detroit Michigan. Currently her company, ADVANCING NURSING LLC, is focused on creating empowered work environments for healthcare practitioners through the acquisition of greater skills, knowledge and process improvement. Vollman is a subject-matter expert for prevention of VAE, CAUTI, CLABSI and HAPI as well as sepsis recognition/ management and the culture of safety for American Hospital Association and the Michigan Hospital Association. In 2012, she was appointed to serve as an honorary ambassador to the World Federation of Critical Care Nurses and is currently the financial director.
