During this program, our speaker will discuss the structure of a best-practice ED case management model and the processes the roles should follow. She will also discuss outcome measures that will ensure your ED program is making a difference for your organization.

Toni G. Cesta, Ph.D., RN, FAAN is a founding partner of Case Management Concepts, LLC, a consulting company that assists institutions in designing, implementing and evaluating case management models in the acute care, emergency department and outpatient settings.  

Dr. Cesta writes a monthly column called "Case Management Insider" in the Hospital Case Management newsletter in which she shares insights and information on current issues and trends in case management.

2. Discuss the role of the social work ED case manager and how to identify social determinants of health. 
3. Review ways in which to “triage” patients in the ED from a case management perspective.
4. Describe EMTALA and how it is applied.
5. Evaluate the value-based reimbursement metrics most greatly affected by ED case management.
6. Apply the case management roles to the ED setting.
7. Discuss ways in which to measure the outcomes of an ED case management model.
