Case Management Across the Continuum of Care: Acute Care and Community Case Management Finally Meet!

Course Curriculum
Integrating case management across the continuum of care requires that hospitals and post-acute providers think like an accountable care organization (ACO), even if they are not one. This kind of thinking requires that patients remain at the center of the wheel around which all case management processes revolve. Case management departments must ensure that they provide an infrastructure for managing patients across the continuum that includes processes for identifying and managing high-risk patients, regardless of setting.

This program will teach you how to coordinate the patient’s transition from health care systems to other settings, such as moving from the hospital to rehabilitation and home settings. There will be a discussion on how to involve the patient and family in decisions regarding care. Factors that affect the integration of case management among and between providers will also be covered, as well as best practice strategies for ensuring that patients do not fall through the cracks.



Learning Objectives
At the conclusion of this session, participants will be able to:
1. Understand how case management is applied in different settings across the continuum.
2. Discuss the vital role of case management in coordinating care across the continuum.
3. Identify strategies for eliminating barriers affecting care transitions.

Toni G. Cesta, Ph.D., RN, FAAN is a founding partner of Case Management Concepts, LLC, a consulting company that assists institutions in designing, implementing and evaluating case management models in the acute care, emergency department and outpatient settings. 
Dr. Cesta writes a monthly column called "Case Management Insider" in the Hospital Case Management newsletter in which she shares insights and information on current issues and trends in case management.
